Queensland Consolidated Acts(1) An application for a licence must—
(a) be made to the chief executive; and
(b) be in the approved form; and
(c) be accompanied by—
(i) the fee prescribed under a regulation; and
(ii) if the application is for a licence under which a fumigation activity may be carried out only in a certain site environment—a declaration of assessment.
(2) A declaration of assessment must be—
(a) in writing; and
(b) signed by an accredited assessor.
(3) In this section—
accredited assessor, for the demonstration of the competence of an applicant for a licence to carry out a fumigation activity in a certain site environment, means a person authorised by a registered training organisation to certify the applicant's competence.
declaration of assessment, for an applicant for a licence under which a fumigation activity may be carried out only in a certain site environment, means a declaration that the applicant is competent to carry out the fumigation activity in the site environment.
