South Australian Consolidated RegulationsAn appeal under section 46 of the Act must be instituted by lodging with the
Minister a notice, signed and dated by the appellant, setting out—
(a) the
full name, telephone number and address for service of the appellant; and
(b)
details of the decision being appealed against, including—
(i)
the date of the decision; and
(iii)
details identifying the registration the subject of the
decision; and
(c)
details of the grounds on which the appeal is based.
