Division 1 — Reporting organisations
1. The provider as defined in paragraph (c)
of the definition of that term in the Health and Disability Services
(Complaints) Act 1995 section 3(1).
2. The Disability Services Commission referred to
in the Disability Services Act 1993 section 6.
3. A hospital board constituted under the
Hospitals and Health Services Act 1927 section 15, or the Minister
in relation to any public hospital controlled by the Minister under the
Hospitals and Health Services Act 1927 section 7.
4. Any other person or body prescribed for the
purposes of this Division.

Division 2 — Other organisations
5. A person or body providing a service to others
under a contract with a reporting organisation.
6. Any other person or body prescribed for the
purposes of this Division.
