appears on any panel shall be summoned by the summoning officer by summons in
or substantially in, the prescribed form which shall, except where this Act
prescribes a shorter period, be served 5 clear days at least before his or her
attendance is required, by the delivery of the summons to the juror in
accordance with section 33.
