union for exemption from the application of section 57 of the Act in
respect of an election shall be made by the Secretary or President of the
union or association in writing stating the grounds for the application and
shall be accompanied by a statutory declaration to the effect that the person
making the declaration is the person authorised to make that application and
further that the information contained in the application is true and correct.
